After 39 years of service to the uninsured, low-income residents of Greater Richmond, the Center for Healthy Hearts (originally the Richmond Area High Blood Pressure Center) will be closing its doors on May 30, 2022. The staff and volunteers will be taking the next 5 months to meet with all of our active patients to help facilitate a transfer to a new medical home of your choosing. We are saddened to be closing our doors but are grateful that we have the opportunity to take care of our patients one final time.

FOR CURRENT PATIENTS

The staff and volunteers will be contacting all of our patients to bring them in for an exit visit. During this visit we request that you bring your current household proof of income so that we may determine for which clinics you are eligible. After you choose your new provider, the staff will assist in making the connection, setting an appointment, and transferring your medical records. We want to let you know that we will be here for you during this transfer time. We thank you for entrusting your health care needs to us and will miss you all.
